Form Builder Components - Assessment

Use the assessment component to Pass and Fail form items with picture and comments. Pre-made true or false.

Dashboard > Settings > Modules > Form > Create Form Builder Form


Permissions

Always remember to ensure the correct people have the appropriate permissions to access your company and project data. This can be done per individual or by applying Permission Templates.

In order to access Form Builder, users will need Edit permissions for Settings


Use the Assessment Component to ask assessment questions while also allowing for additional contexts like photos and comments. For example, the question on a form such as, "Is there visible damage to the equipment?" can be followed up with a photo and the user's comments using the assessment component.

Creating Assessment Component

To create an assessment, begin by dragging and dropping the component into the form builder template. Then click on the pencil icon to edit the functionality.

Once you have clicked the "edit" you will be provided with the following fields that can be edited.

  • Label: Under Label, type your question.

  • Assessment Options: You can exclude the ability to add photos or comments to the response section.

  • Required: Decide if this is a required field that must be filled before a user can save the form.

  • Show Component: Pick if this section should be present on the saved PDF.

  • Title-based Visibility: Allows for the Assessment to only be completed by a user with a specific Title.

Users will be prompted to select from True (Checkmark), False (X), and Not Applicable (N/A). They will also be able to add images by clicking the camera icon and comments with the chat bubble icon to add additional details.
